Please complete this form to request that a representative from the chancellor's office attend and/or participation at an event. Once completed, please click the SUBMIT button at the bottom of this form and it will be emailed to Jodi Boese in the Chancellor's Office.

Note: This form should be submitted at least 3-4 weeks prior to the meeting or event unless it is an emergency or special called meeting.  If under these circumstances, or if you have any additional questions, please contact Jodi Boese in the Chancellor’s Office at 618-453-1373 or via email at, prior to submitting the form.
